• FERPA Authorization to Release Form
• General Petition Form (First year students requesting schedule change due to compelling circumstances; part-time upper-division students requesting more than 11 units, requests not otherwise listed.)
• Independent Study
• Leave of Absence (Medical Leave of Absence Requires Meeting with the Vice Dean for Academic & Student Affairs)
• Request to Reschedule Examination (Requests for Disability Accommodations Require Meeting with the Assistant Dean of Student & Diversity Services)
• Request to Switch Divisions
• Visit or Take Graduate Courses

Petitions may also be picked up from the Registrar and the Office of the Vice Dean for Academic & Student Affairs.
